Position Summary
The Case Manager provides direct support to individuals and families dealing with behavioral health challenges. The Case Manager will coordinate care, connect clients with essential services, and ensure they receive the resources needed to work toward stability, recovery, and improved quality of life. This role is key in ensuring that clients—especially youth and families—receive person-centered and strengths-based support in alignment with treatment goals.

Key Responsibilities
• Provide individualized, culturally sensitive case management services to clients and families
• Develop, implement, and monitor client service plans in collaboration with clinical staff
• Assist clients in accessing behavioral health services, housing, employment, education, and community resources
• Conduct regular home visits, school visits, and in-office appointments to assess progress and address client needs
• Maintain accurate and timely documentation in compliance with Medicaid and agency standards
• Participate in team meetings, supervision, and multidisciplinary case reviews
• Advocate for clients’ needs across systems (mental health, education, juvenile justice, child welfare, etc.)
• Build collaborative relationships with schools, courts, service providers, and families
• Support crisis intervention efforts and safety planning as needed
• Ensure services are delivered in a trauma-informed, strengths-based, and client-centered manner

Qualifications
• Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s preferred), a high school diploma and 3 years experience may be considered.
• Minimum of 1–2 years of experience in case management, social services, or behavioral health
• Knowledge of community resources and systems that impact youth and families
• Strong written and verbal communication skills
• Ability to manage a caseload and maintain organized documentation
• Familiarity with Medicaid billing and behavioral health documentation standards (preferred)
• Valid driver’s license and reliable transportation required

• This position requires flexibility to travel locally for client visits and community meetings
• Occasional evening or weekend hours may be required

Position Summary
The Therapeutic Mentor provides structured, goal-oriented support to youth and young adults experiencing emotional, behavioral, or social challenges. Working closely with clinical staff and families, the Therapeutic Mentor serves as a positive role model, helping clients build self-esteem, develop coping skills, improve relationships, and work toward treatment and life goals in real-world settings.

Key Responsibilities
• Engage youth in one-on-one or small group sessions that promote emotional growth, positive behavior, and healthy decision-making
• Support clients in developing social, communication, and life skills through structured activities and mentoring relationships
• Reinforce strategies and goals set in clinical treatment plans in collaboration with therapists and case managers
• Provide community-based, school-based, and home-based support as appropriate
• Help clients navigate challenges related to school, peer relationships, self-esteem, and behavior
• Serve as a positive role model and advocate for clients, fostering trust and resilience
• Maintain professional boundaries and confidentiality at all times
• Document sessions and progress in accordance with Medicaid and agency standards
• Attend team meetings, supervision, and training sessions as required

Qualifications
• Associate’s or Bachelor’s degree in Psychology, Social Work, Education, or a related field (or relevant experience working with youth), a high school diploma and 3 years experience may be considered
• Experience working with youth who have behavioral health or emotional needs
• Ability to build rapport with children, teens, and young adults from diverse backgrounds
• Knowledge of trauma-informed and strengths-based approaches
• Excellent communication and problem-solving skills
• Reliable transportation and valid driver’s license required
• Ability to work flexible hours, including evenings and weekends if needed
