2025 Fatherhood Revisited Referral Restructure


Fatherhood Revisited is working to improve our referral process. We are noticing that we have long wait times from referral to assigning a therapeutic mentor or case manager and we are working to fix our system.


Going forward, if you plan to refer a client, please read the following:


1. We need a mental health assessment or DAF to be submitted with the referral. If a referral is submitted without a DAF and we are not able to schedule an assessment with the guardian after 3 attempts (or have 2 no shows), the referral will close.


2. If the person referring the client does not have a mental health assessment or DAF and would like Fatherhood Revisited to complete it, the guardian and client must complete the DAF before Fatherhood Revisited will complete an official intake. We will email the guardian and person submitting the referral to schedule the assessment. If Fatherhood Revisited has reached out to the guardian to schedule 3 times without a response, the referral will close.


3. If the person referring the client submits a referral without other needed information and Fatherhood Revisited has reached out to the person referring the client to get the information without a response, the referral will close.


4. If Fatherhood Revisited has completed an intake and the guardian has not completed the intake paperwork in full, we will reach out 3 times. After the third attempt, the referral will close. We enjoy serving the community at large and our youth.


We also recognize the need for change, so we have to institute this new system. We look forward to continuing to serve you in the future.
